Requirements to Participate:

1. A signed parental consent statement
2. Copy of a physical, dated within 6 months-1 year of camp - Physician’s statement is acceptable in lieu of a physical
3. Copy of the front and back of camper’s health insurance card - **NO CAMPER WILL BE PERMITTED TO PARTICIPATE UNTIL ALL REQUIRED MEDICAL INFORMATION IS RECEIVED!**
4. 4) Balance must be paid in full.
